About the Role
As Case Manager, you will be responsible for developing and implementing Case Plans for children and young people in Out of Home Care. While drawing from your tertiary qualifications and experience, you will have a demonstrated understanding of current case plan practice for children and young people, experience with working with children and young people in out of home care, have strong negotiation and mediation skills and a current driver’s licence.
Key Responsibilities
- Develop and implement high quality case plans for children and young people
- Develop and maintain supportive relationships with children in out of home care
- Ensure the CIRTS database is up to date with all client and carer information
- Provide support and supervision to Foster Carers
- Support and monitor placements
- Participate in on call roster as required
- Ensure compliance with all internal policy and procedure, statutory and contractual requirements
Skills & Experience
- Degree in Social Work, Social Science, Psychology or Human Services
- Demonstrated understanding of current casework practice for children
- Demonstrated experience in working with children in out of home care and foster carers
- Demonstrated ability to develop quality case plans
- High level organisation and time management skills, and ability to reorganise work priorities to meet changing demands
- Demonstrated ability to work with individuals, families or groups from strengths based culturally appropriate perspective
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Tasmanian Working With Vulnerable People Registration.
